This version of the Collect app is configured with forms for antenatal care, which communicate with the live demo of Medic Mobile. To get started set the "Own phone number" in the "General Settings".

Medic Mobile's Collect app allows health workers or managers to send reports to Medic Mobile. The app includes easy-to-use forms with skip logic, location data, in-app data validation, and multimedia support. Reports can be submitted using data (mobile internet, or wifi) as well as SMS.
Collect extends ODK Collect, part of the Open Data Kit toolset. Forms and settings can be configured for specific projects, and easily be updated in the field. Alerts can notify health workers when reports need to be submitted.
Using Collect, health workers can register people into health programs, screen for high-risk pregnancies, submit stock reports, report disease outbreaks, and more.
